Terms of Service

These Terms govern your use of the MI Health application. By creating an account or using the app, you agree to these Terms.

1. The service

MI Health is a tool for storing your personal health information and sharing the medical portion of it with health-care providers you authorize. It is an information-management tool only.

Not medical advice. MI Health does not provide medical advice, diagnosis, or treatment, and is not a medical device. Always seek the advice of a qualified health professional with any questions about a medical condition. In an emergency, call 911 or your local emergency number. Never rely on MI Health for emergency medical information.

2. Eligibility

You must be at least 16 years old to use MI Health and be able to form a binding agreement.

3. Your account

You are responsible for keeping your login credentials secure and for the accuracy of the information you enter. Keeping your health information accurate and current is your responsibility.

4. Granting provider access

You control which clinics can access your records. When you grant access, that provider may view and update the medical portion of your profile until the grant expires or you revoke it. You are responsible for granting access only to providers you trust; we are not responsible for a provider's use of the information you choose to share, or for the accuracy of edits they make.

You may stop using the service and delete your account at any time from within the app. We may suspend or terminate access if you violate these Terms or to protect the service and its users.

7. Disclaimers

The service is provided "as is" and "as available", without warranties of any kind, whether express or implied, to the fullest extent permitted by law.

8. Limitation of liability

To the maximum extent permitted by law, MI Health and its operators will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of data, arising out of or relating to your use of the service.

9. Governing law

These Terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable there, without regard to conflict-of-laws rules.

10. Changes

We may update these Terms from time to time. Continued use of the service after changes take effect constitutes acceptance of the revised Terms.
